Welcome to the City of San Bruno! We look forward to you joining our team of professionals. You will have an appointment with Human Resources (HR) on your first day. The following is a list of new employee paperwork that you can print/complete at home, or in HR on your first day. Please remember to bring all supporting documentation to your meeting with Human Resources, i.e.:

Training
- ICS 100 and ICS 700 Training Certificates - Under California Government Code Section 3100-3109, all persons employed by the county, city, state agency or public district in the State of California are obligated to serve as Disaster Service Workers. Please Watch the Video, then take both the ICS 100 and ICS 700 modules. You can take the tests multiple times. Print the completed ICS 100 and ICS 700 certificates and bring them with you on your first day.
- Preventing Workplace Harassment Training - Per SB1343 all employees must undergo anti-harassment training (1 hour for non-supervisors and 2 hours for supervisors). HR will set up the appropriate online training for you upon hire.
- Mandated Reporter Training - If you are considered a mandated reporter through your job classification -or- if your job duties as an employee with the City of San Bruno include interaction with children or other sensitive populations, please do the following:

Health Plans
The City offers a comprehensive health plan including medical, dental, vision, and prescription benefits that meet the Affordable Care Act Standards and the Individual Mandate Set by the IRS (PDF).
Please complete the Master Enrollment Form (PDF) to enroll in healthcare benefits. If you will be adding dependents, bring a marriage certificate to add your spouse and birth certificates to add your children. The City also extends health benefits to domestic partners (Domestic Partner Affidavit (PDF) must be signed and notarized).
Please choose your plan options carefully as you will remain in the plan for 12 months. After 12 months, you may participate in the City's Open Enrollment period, when you can switch health plans and/or add additional dependents (with the exception of status changes such as marriage or newborns, which can be processed at the time of the status change).

Opt-Out
Employees who can show proof of coverage in a qualified medical plan through a spouse or parent have the choice to opt-out of the City's health plan in order to receive a monthly benefit amount:
- Fill out the Master Enrollment Form (PDF) and check the box marked "OPT OUT (MEDICAL PLAN)"
- Provide Proof of Coverage (letter from employer or printout)
- Complete the Right to Decline Coverage Form (PDF)
Keycard Access [City Hall Employees Only]
- Please make your choice (either app or keycard) using the following form:
- City Hall doors have a touchless entry system that operates via Bluetooth technology. You have a choice between utilizing a mobile application downloaded onto your City or personal device (if chosen for convenience) or a City-issued key card. This system will replace physical keys and numeric keypads. The purpose of the new system is to provide touchless entry and enhance building security during and after business hours for employees.
- Please note that employees are responsible for safeguarding their key card or smart phone application, including the following responsibilities: City-issued key cards and smart phone applications are provided to the named employee ONLY and are not to be shared. Key cards are to be returned when an employee leaves City service. Employees must ensure that key cards and mobile phones are in their possession at all times. Report lost or stolen key cards or mobile phones to IT immediately via email at ITSupport@sanbruno.ca.gov. Employees must ensure that the door is locked after exiting the room. Do not prop building or room entry doors open at any time. California Penal Code Section 469 prohibits the unauthorized making, duplicating, use, or possession of keys to public buildings.

Deferred Compensation Savings Plans
Deferred Compensation Savings Plans (also known as "457" plans) allow employees to designate pre-tax contributions into an investment account for additional retirement savings. Pre-tax contributions reduce your taxable income for the year. Contributions (and associated investment earnings) are then not subject to tax until you withdraw them.
The City of San Bruno offers two choices for Deferred Compensation plans. Employees may start or stop contributions at any time. To enroll, please submit a completed enrollment form to Human Resources.

Supplemental Life Insurance
The City offers the option for you to add Supplemental Life Insurance (PDF) for yourself and/or your spouse. To apply, please complete the Evidence of Insurability (EOI) Form (PDF). Amounts above the Guaranteed Issue (GI) must be submitted for approval.
Workers' Compensation Benefits / Pre-Designated Physician
Employers are required to provide workers' compensation benefits to employees who are hurt on the job. Workers Compensation (PDF) claims are evaluated by SFO Medical Clinic, unless a Pre-Designated Physician (PDF) is on record with Human Resources 30 days prior to work-related injury or illness.
