You may download a PDF of the Service Application form and either fill it out online (use the form fields in Adobe Acrobat, then print it out) or just print it out and fill it out by hand. 

Please note that we will need to see a copy of your lease agreement if you do not own the dwelling.

Submitting Your Application

It is preferable for you to bring your service application to our office to finalize this form and present a photo ID.

Emailing your personal information is not recommended.  Emails are not secure.

Automatic Payments

If you'd like to set up automatic payments for your CityNet bill, please complete and submit the Automatic Payment Request Form (PDF) with your service applications.
